Completing a Work Order Form
- Fill out a Telecommunications work order form to submit requests for services. If you need a work order mailed or faxed to you, call us at 979.845.1020.

Telecommunications assigns a work order number to your request and sends an email confirmation within 24 hours (or one working day). If you do not receive the work order number within this time, please contact us at 979.845.1020.
Required Information for a Telecommunications Work Order
Requesting Department: Enter the name of the department requesting the service.
Service Request Coordinator: Enter the name of the individual responsible for providing clarification about the service requested, and the contact person for the telephone company installation/service representative.
Phone Number: Enter the service request coordinator's telephone number.
Work Order Number: Leave this space blank. This number is issued by Telecommunications.
Estimate Only: If requesting only a cost estimate, enter 'yes'. No service will be performed until approval is obtained.
Mail Stop: Enter the four-digit departmental mail stop number.
Email Address: Enter the service request coordinator's email address.
System Part Code: Enter the two-digit code number that identifies which part of the A&M System the requesting department is assigned.
Centrex Account Number: Enter the authorized Centrex account number to which the requested service will be charged. The cost for service will be included in this account's Centrex bill. Be sure to include the last five digits of the Centrex account number.
When requesting service for an existing telephone number, use the account number to which that telephone number is being charged. If changing an existing service from one Centrex account number to another, indicate this in the remarks section of the work order form.
Account Name: Enter the account name for the authorized Centrex account.
Building Location: If the building is not located on campus, enter the street address. If the building is on campus, enter Main Campus, Riverside Campus, etc.
Building Name: Enter the assigned building name in which requested service is to be provided. If no building name is assigned, enter the functional use of the building.
Building Number: Enter the assigned building number in which the requested service is to be provided. If you do not know the building number, you can look up your building on the Campus Map, or contact Telecommunications at 979.845.1020 or your area maintenance shop for assistance.
