Service Requested Section for General Work Orders
The following provides assistance in filling out the Service Requested section for the general work order. Other work order forms have different Service Requested sections.
Item #: This entry serves to identify a specific request for service on this work order.
Add/Move/Change/Disconnect:
- Add: Enter "Add" to request new service.
- Move: Enter "Move" to request move of existing service from one location to another.
- Change: Enter "Change" to request change in existing service such as change in calling access, pick-up group, station equipment, etc.
- Disconnect: Enter "Disconnect" to request removal of all existing service for a specified line or circuit number.
Line or Circuit Number: Enter the applicable telephone line or dedicated circuit number on all move, change, or disconnect service requests.
Intercom Sta Number #: Enter the applicable intercom station number on all move, change, or disconnect service requests associated with multi-line phone systems.
From Location Bldg/Room: Enter the exact location of existing service on all move, change, or disconnect requests.
To Location Bldg/Room: Enter the exact location to which service is to be moved or added on all add, move, or change service requests.
Voice Mail: Select voice mail service, if applicable.
Maximum Calling Access: Enter maximum calling access authorized on all telephone line add or change service requests. See more about Entry Opions For Maximum Calling Access.
Pick-Up Group: Enter an existing telephone now in the pick-up group with which this add or change service request should be associated. To initiate a new pick-up group (*61), list all numbers to be placed in the new group in the Remarks section.
Station Equipment Types: Enter a description of station equipment type on all service requests. See the following for entry options. If you would like to change equipment, IE: SL desk to SL Meridian, please use the Remarks section to show changes. Note: you can save money by exchanging your phones at the Phone Mart. See tips for saving money and time.
- For a Single Line Desk Instrument, enter "SL Desk".
- For a Single Line Wall Instrument, enter "SL Wall".
- For a Single Line Speakerphone, enter "SPKPH".
- For a Single Line Meridian Phone, enter "SL Meridian".
- For a Two Line Desk Instrument, enter "2L INST".
- For a Two Line Meridian Phone, enter "2L Meridian".
- For a Multi-line Key System, enter "ML KSYS" and describe in the Remark section of the work order.
- Jack Only (User Owned Equipment), enter "JK Only".
- Line Status Monitor (10 or 20 Lines) ,enter "LSM 10" or "LSM 20".
- For other, enter "Other" and describe in the Remarks section of the work order.
Remarks: Reference Item # to which the remarks applies. Enter remarks as needed to clarify any request for service. The Remarks section may also be used to describe service requests which do not logically fit the standard work order format. This section should also include any installation restrictions (physical, date, and/or time constraints). Attach additional sheets as needed. Sketches or drawings are particularly helpful.
